• We do not provide medication refills for patients we have never assessed. If you are in need of a refill you must go through your current prescriber or pharmacy. Once established in our office, we can discuss medication management with you.

  • Check insurance coverage. Currently, we contract with Optum/UMR/UHC/Aetna/Cigna/BCBS.

  • Currently, we do not contract with Tricare, Medicare, or AHCCCS plans.

  • *We do not take self-pay for patients covered by Government plans such as AHCCCS*

  • Blue Cross/Blue Shield: We process BCBS through HEADWAY. You can check your coverage here: https://headway.co/insurance/form

  • Self-Schedule online. Due to the high demand for appointments, all scheduling is done online. Please choose a date/time and submit an appointment request HERE

  • We do not need records from your previous provider to complete a new patient intake. Once you meet your new provider, if more information is required, we will discuss the process with you.

    Please note that our office is an independently owned private practice with separate policies and procedures from your referring provider.
